Rib Flare and Its Impact on Spinal Stability and Breathing

Rib flare refers to a posture where the lower ribs protrude forward and upward instead of sitting down and in, aligned over the pelvis. It’s commonly seen in people with poor postural control, athletes who overuse their back extensors, or individuals with inefficient breathing patterns.

How Rib Flare Affects the Lower Back

When the ribs flare forward, they usually go hand-in-hand with an anterior pelvic tilt. This shifts the body into excessive lumbar lordosis (arched lower back). To stay upright, the body compensates by over-activating the lower back muscles while the deep core muscles (especially the abdominals) become underactive. Over time, this imbalance increases stress on the lumbar spine and can contribute to persistent low-back pain and stiffness.

Impact on Breathing

Rib flare significantly alters normal breathing mechanics. Instead of the diaphragm moving down and the rib cage expanding to 360, breathing becomes more chest dominant. The diaphragm stays in a shortened, inefficient position, reducing oxygen intake and increasing reliance on accessory neck and chest muscles. This can lead to shallow breathing, quicker fatigue, and poor core stability—because the diaphragm is a key stabilizer of the spine.

Correction with Physiotherapy, Exercises, and Breathing

At Pure with Physiotherapy, we focus on restoring rib-pelvis alignment and improving breathing mechanics with Postural re-education, Manual therapy, breathing retraining, core activation targeting deep abdominals (transversus abdominis), mobility and strengthening.

With consistent practice, proper breathing and controlled exercises help bring the ribs down, reduce excessive lumbar arching, improve spinal stability, and make movement more efficient and pain-free. Correcting rib flare isn’t just about posture—it’s about restoring the natural relationship between breathing, core control, and spinal alignment.
